For images that do not include “reprinted with permission” or similar wording, we suggest this format: 

Image Producer Last name, first initial. Name of image: Where it appears on the curriculum (lesson or question bank title). In. [Editors.] Name of Curriculum. Which edition. University of Washington Infectious Diseases Education & Assessment Program. Updated: [when unit was last updated]. Accessed: [when image was seen on website]. Link to the Quick Reference version of the unit.

For Example:
